Clarifications on some structures/elements

I would like to confirm definition of some strctures/elements. For these question, please refer to
 
(A) file name: SIS P3 - Misc IS-XML Standard - V2
(B) tab: 2)Charge Catgry-Charge Code Map
 
- Grd Haldg-Catering
In a current invoice, normally cabin class and a list of meal types (normal/vegetarian/child meal etc) and respective numbers are listed. "Cabin Class" itself is a field required, how about meal types?
 
- Mail
In a current invoice, normally the "route" of the mails i.e. from where to where is listed down by line item. Is this captured as a field required?
 
- Mishandled pax and bagg
Currently there are pages of supporting docs (tags etc) billed, due to invoice verification/audit/legal needs of various airlines? Any chance to resolve this issue without paying extra fees on attachment?

The software IATA uses doesn't allow me to shift to other forums. This should be in the Record Structures forum.
 
More importantly, it is very important that, as per the instructions, you ask these as three separate conversations. It will be easier for others to browse through, and also to let the conversations take 3 different paths (e.g. one might go into a change request).
