Form C coupon breakdown mandatory
ISPG Section 3.8 Sampling UAF
Reason Type - Sampling Form C Reasons
The chart in the section indicates that coupon breakdown is mandatory.
Does this mean that in addition to the PAX - Sampling Form C Record, which includes a ticket number, coupon number, and reason code, that the PAX Coupon Record must be resubmitted to SIS on the IS-IDEC record? Or is additional detail records needed since the PAX Coupon record should already exist in SIS from the prime billing (Form A) submission by the billing carrier? |

The mandatory coupon breakdown in the case of Form C is the PAX Sampling Form C Record. No other breakdown information required to support Form C billings.
