Please note that the fillable form of the "Shipper's Declaration" requires Adobe Acrobat (c) software in version 6 or 7. When using the free "Acrobat Reader" version (which can be downloaded here) the "Save file" functionality will not be available.

Effective January 1st, 2007 the Shipper's Declaration forms with the "Proper Shipping name" first were no longer permitted.
