Safety Report 2008

The recently released 45th Edition of the Safety Report contains in-depth analysis of 2008 accidents.  It focuses on current issues such as runway excursions, Controlled Flight Into Terrain (CFIT), loss of control in-flight and tail strikes. Here is a look at last year’s accidents in comparison to the previous year.

Top 5 issues from the Safety Report

Runway excursion prevention: Runway excursion was the most frequent type of accident in 2008, accounting for 25% of accidents. IATA will launch the Runway Excursion Risk Reduction (RERR) Toolkit in the third quarter of 2009, which will address runway excursions.

Ground damage reduction: Ground damage was the second most predominant type of accident. Following the launch of the IATA Safety Audit for Ground Operations (ISAGO) in 2008, IATA will conduct 80 ISAGO audits in 2009.

Safety Management System (SMS) implementation: In almost one-third of accidents last year, deficient airline safety management was a contributing factor. IATA will deliver new guidance material on Safety Management System (SMS) implementation in September and continue to assist airlines with SMS implementation at an individual and a regional level.

Safety Enhancement in Maintenance Operations: 15% of accidents in 2008 involved maintenance events. IATA will launch a revised safety strategy in 2009, which includes a focus on safety management in maintenance operations.

Regional Safety Issues: Operators based in the Commonwealth of Independent States (CIS) and Latin America and the Caribbean, had the highest regional accident rates in 2008. IATA is actively involved in the regions. A Memorandum of Understanding has been signed with the Russian authorities and strategy has been developed to address safety concerns in the region under the umbrella of the Global Aviation Safety Roadmap.

Safety Management System (SMS)

IATA assesses SMS implementation of members

A total of 44 airlines participated in an IATA survey on the level of SMS implementation. The survey was structured on four components and 12 elements of the International Civil Aviation Organization ( ICAO) SMS framework.  A gap analysis was conducted on these components and elements.

The goal of the analysis was to determine areas where member airlines need most assistance, in order to successfully implement an SMS. The analysis was conducted on global, industry-wide level as well as on regional levels of SMS compliance.

Top Findings

At an industry-level:

  • "Safety assurance" was the component of the SMS that had the highest number of non-conformities.
  • "The management of change" was the element of the SMS that had the highest number of non-conformities.
On a regional level:

  • Asia/Pacific was the region where SMS implementation is at its lowest.
  • With regards to specific elements, these varied per region.

The results of this gap analysis are intended to assist IATA and its SMS task force to develop best practices to address the findings and benefit the members.

STEADES

STEADES achieves major milestone

With nearly 90 airlines participating, the IATA Safety Trend Evaluation, Analysis & Data Exchange System ( STEADES) now represents three million sectors annually.  This equates to approximately 10% of western-built jet airline traffic and much added value to STEADES ability to offer benchmarks to the industry.

Recently, STEADES has published an analysis on Lightning Strikes and ILS Signal Reliability.

IATA Operational Safety Audit (IOSA)

Successful compliance of all IATA airlines registered by 31 March 2009

At IATA's Annual General Meeting, seven airlines were commended for their early support and participation in the IOSA Programme:

  • American Airlines in North America
  • Croatia Airlines in Europe
  • Kenya Airways in Africa
  • LAN Airlines in South America
  • Qatar Airways in the Middle East
  • Singapore Airlines in Asia Pacific
  • Xiamen Airlines in North Asia

Congratulations to these airlines and to all IOSA registered airlines, evidencing their commitment to safety.

IOSA Audits Update

By 31 March 2009, IATA achieved an historic objective – all members were IOSA registered.
In total more than 700 audits were conducted and nearly 1200 audits were avoided.

IATA Safety Audit for Ground Operations (ISAGO)

ISAGO audits update

Aeroflot and its subsidiary Airport Moscow were audited in late May at Sheremetyevo International Airport in Moscow, making the first ISAGO Audits in Russia.
In total, 51 audits have been conducted since the launch of the programme last year (24 headquarters audits and 27 station audits)

Air India, KLM, Korean Air, Iran Air and Mexicana join the ISAGO Pool

Five airlines joined the programme between April and May, increasing the number of ISAGO Pool members to 35 airlines.  The Pool consists of 134 ISAGO qualified auditors.

ISAGO in Brussels

On 6 May, IATA delivered an ISAGO information session at Aviapartner’s head offices in Brussels in front of 40 delegates from Aviapartner staff around Europe and from their customer airlines.
Following this seminar, the ISAGO oversight committee bi-annual meetings took place on 7-8 May at Eurocontrol headquarters involving over 30 members from participating airlines, ground handlers, regulators and international organisations.

A fast growing registry

In early June, 13 providers were already on the ISAGO Registry at 13 different locations, from Bogota to Dubai to Hong Kong.
