  6. E-TIN (Tax Identification Number) certificate in the name of the Company


If business entity: Partnership
  1.   Notarized copy of registered Partnership Deed
  2.  Valid Passport copies or National ID Card of each partner 
  3. E-TIN (Tax Identification Number) Certificate
  4. Signed Application Summary Form with the signatory of all the partners

If business entity: Sole Proprietor
