Please use the following mandate and bank letter (Direct Debit Mandate and bank letter).
Once both forms are fully completed and signed (bank mandate and bank letter), please send these documents to your bank in order for them to set up the account and stamp the mandate as being received. Once done, bank sends a copy of the stamped mandate to IATA to the following address:
