Accident Investigations

Aviation accident investigation is a cornerstone of global aviation safety. Its primary purpose is not to assign blame or determine liability, but to uncover what happened, why it happened, and how future occurrences can be prevented. 

At the heart of the international investigative framework is ICAO Annex 13 – Aircraft Accident and Incident Investigation. Annex 13 sets out the global standards and recommended practices that ensure investigations are conducted independently and objectively. It emphasizes the importance of maintaining investigative integrity, protecting sensitive information, and promoting the open exchange of safety data.

A key component of Annex 13 is the requirement for States to communicate safety information in a timely manner. To support global learning and transparency, States are expected to:

  • Publish a Preliminary Report within 30 days of an accident.
  • Issue annual Interim Statements if the investigation remains open.
  • Release a Final Report as soon as possible, ideally within 12 months.

These timelines help ensure that emerging safety lessons are shared promptly with the worldwide aviation community, enabling operators, regulators, and manufacturers to take proactive action before similar conditions materialize elsewhere.

By adhering to Annex 13 principles, States strengthen international collaboration, enhance safety culture, and contribute to a global aviation system built on learning, accountability, and continuous improvement.
